This form is for students who have received orders for active duty during the academic semester.
When filling out the form, indicate whether you intend to withdraw from school or take an incomplete in each of your classes. The completed form notifies the University Registrar of your intentions. You should also notify all of your professors and other instructors.
After you have submitted this form, please submit a copy of your orders to the MU Veterans Center and the University Registrar.
